GUIDE IN ACCOMPLISHING THE DAILY ATTENDANCE REPORT

1          Duplicate this spreadsheet based on the number of sections you are handling.

2          Fill out the upper field of the report (learning area, year & section, faculty member, class schedule, and ICLEx schedule) and learners' names for the month of JANUARY only.  These will automatically reflect for the other months.

3          For learners' names, delete (not hide) the blank rows, as these will still be counted for the total number of students per day.

4          School days for each month are already indicated.  There is no need to change the dates.

5          Indicate "X" for absent and "T" for late student each day. Please attach your e-signature in each file/sheet.

6          The spreadsheet automatically computes the the total number of absences and tardiness.  Note that three (3) tardiness is equivalent to one (1) absence.

7          Monitor the number of absences of students.  Students are allowed 14 hours of absences for the whole term.  Subject teachers should inform the adviser of the seventh (7th) hour of absence.  The adviser must meet with the parent to inform and properly address the student's absences.

8          Beyond 14 hours of absences merit a grade of FA (failure due to absences).

9          At the end of the term, kindly upload the report for all handled sections using this Google Form link: https://forms.gle/axkkdVaJAp1SNHEg8

10 Save the file using this format: Grade&Section_DAR_Subject_Name of Teacher (e.g. 12-ABM1_DAR_Business Finance_Redelyn Yumul)
