Data Privacy Consent Form
In compliance with
the Data Privacy Act of 2012 (RA 10173) and its Implementing Rules and
Regulations, you are allowing UPD JSWAP and UPD SW 220 Students to store
and use your personal data, guided by the following conditions:
1. The attendee's
personal data, which include their name, and email address will be used ONLY
for the purpose of communicating for the Webinar entitled Extraordinary Social Workers in Extraordinary Times: Challenges, Self-Care Strategies and Success Stories from Social Work Practice at the Time of the Pandemic.
2. UPD JSWAP and UPD SW 220 Students will take the necessary precautions in using
and protecting all personal information.
The information
collected will be stored in the official database of UPD JSWAP and UPD SW 220 Students and accessible only to the its officers/members . UPD JSWAP and UPD SW 220 Students shall not disclose any personal information to any other party without the informed verbal, written of electronic consent of the person/s concerned. The UPD JSWAP and UPD SW 220 Students shall retain the information obtained from the date of submission of the form for a period of one (1) month.
Rest assured that all data collected
will be handled WITH UTMOST CONFIDENTIALITY and will be used SOLELY for the
stated purposes above.
