Participation Waiver – The undersigned agrees to give consent to be photographed and or videotaped during participation in the Team Link\Guardian Self Defense program. I understand that any pictures or video taken may be used fro promotional purposes ONLY (including website, brochures or other promotional material). The undersigned consents to participate in a martial arts program. He/she understand that such training is associated with certain risks including but not limited to musculoskeletal injuries, spinal injuries, and abnormal blood pressure. Every effort will be made to minimize these risks. As with any physical exercise, the undersigned agrees to have a medical exam and approval to participate by a board certified physician if he/she is aware of any possible predisposing conditions that would prevent him/her from participation. In addition, he/she agrees to notify the instructor of any and all ailments prior to, during, or immediately following participation in any class or training session.  Should the undersigned become injured, he/she allows the instructor to contact Emergency Medical Services for proper care. If under age 18, the undersigned agrees to allow the instructor in attendance to act as a temporary guardian for the purpose of emergency medical services and care at any hospital until a parents or legal guardian arrives. The undersigned, their heirs, or estate agrees to hold harmless Team Link\Guardian Self Defense program instructors, fellow students, and affiliates from any liability in connection with participation in this program. The student and/or his parent or legal guardian have read the above and have agreed to the content of the student application. The undersigned also agrees that the instructor may suspend or discharge the student from classes with or without cause at any time.
