Method of giving consent to disclose information by means of the internet.

1. ผู้ให้ความยินยอมจะต้องกรอกข้อมูลให้ถูกต้อง / The data subject must fill out the information correctly.
2. การให้ความยินยอมผ่านระบบอินเตอร์เน็ตให้ถือว่ามีการลงลายมือชื่อแล้ว / Giving consent via the Internet system shall be deemed to have been signed.

ตามพระราชบัญญัติ คุ้มครองข้อมูลส่วนบุคคล พ.ศ. 2562 ได้ให้สิทธิแก่เจ้าของข้อมูลในการยินยอมให้ผู้ควบคุมข้อมูลดำเนินการเก็บ รวบรวม ใช้ หรือเปิดเผยข้อมูลส่วนบุคคลได้ ตามวัตถุประสงค์ที่ระบุไว้ ตามมาตรา 19 แห่งพระราชบัญญัติดังกล่าว                                   According to the Personal Data Protection B.E. 2562(2019) has given the data subject the right to consent to the data controller to collect, use or disclose personal data. according to the objectives stated in accordance with Section 19 of the said Act.
    ทั้งนี้ บริษัทฯ จัดเก็บข้อมูลการสมัครงานของท่าน เพื่อประโยชน์สำหรับการสมัครงานกับตำแหน่งที่บริษัทฯ ต้องการ เท่านั้น โดยบริษัทฯ จะเก็บข้อมูลนี้เป็นระยะเวลา 6 เดือน ซึ่งข้าพเจ้ายินยอมที่จะให้ข้อมูลตามความเป็นจริง ดังมีรายละเอียดต่อไปนี้
    The company stores information about your job application. For the benefit of applying for a job with the position that the company wants only, the company will keep this information for a period of 6 months,  I agree to provide the information truthfully as are the following details:

ข้าพเจ้ายินยอมให้นำข้อมูลส่วนบุคคลของข้าพเจ้าเพื่อประโยชน์ในการสมัครเข้าทำงาน  เก็บ, ใช้, และ/หรือ เปิดเผยข้อมูลส่วนบุคคลที่เกี่ยวข้องกับข้าพเจ้า รวมถึงข้อมูลการตรวจสอบประวัติส่วนบุคคลหรือประสบการณ์การทำงานที่ผ่านมาของข้าพเจ้า เพื่อนำไปใช้สำหรับการสรรหา, การสัมภาษณ์งาน, การว่าจ้าง, และการกำหนดค่าตอบแทน ตลอดจนเปิดเผยข้อมูลดังกล่าวไปยังบุคคล และ/หรือ หน่วยงานต่างๆที่เกี่ยวข้อง เพื่อประโยชน์ในการประกอบการพิจารณาคัดเลือกเข้าทำงานในตำแหน่งงานที่สมัครไว้ / I consent to the use of my personal information for the purpose of applying for employment, collecting, using, and/or disclosing personal information related to me includes information about my personal background check or past work experience.  To be used for recruiting, job interviews, hiring, and compensation as well as disclosing such information to individuals and/or various relevant agencies for the benefit of considering the selection to work in the position.
โดยข้าพเจ้ายินยอมให้เปิดเผยข้อมูลดังต่อไปนี้
I hereby consent to the disclosure of the following information.
1. รายละเอียดข้อมูลต่างๆที่ผู้สมัครงานได้ระบุในใบสมัครงานของบริษัทฯ เช่น ชื่อ-นามสกุล, ที่อยู่, หมายเลขโทรศัพท์, การตรวจสอบบุคคลอ้างอิง/บุคคลที่เกี่ยวข้องตามที่ได้ระบุไว้ในใบสมัคร, ประวัติส่วนบุคคล, ประวัติการทำงาน รวมถึงข้อมูลของครอบครัวของข้าพเจ้า เป็นต้น / Details of various information that the applicant has specified in the Company's job application such as name-surname, address, telephone number, check of references/relevant persons as specified in the application, CV section, person, work history including the information of my family, etc
2. รายละเอียดข้อมูลต่างๆในเอกสารหรือหลักฐานที่ผู้สมัครงานได้นำส่งให้กับบริษัทฯ เช่น สำเนาบัตรประชาชน, หลักฐานเกี่ยวกับที่อยู่อาศัย, สำเนาวุฒิการศึกษา, หลักฐานเกี่ยวกับการทำงานและรายได้, ตลอดจนรูปถ่าย และ/หรือ เอกสารทางราชการใดๆที่ผู้สมัครได้มอบไว้เป็นหลักฐาน รวมทั้งใบสมัครงาน ตลอดจนภาพและเสียงในการสัมภาษณ์งาน เพื่อใช้ในการพิจารณาคัดเลือกขั้นตอนต่อไป / Details of various information in documents or evidence submitted by job applicants to the company, such as a copy of ID card, proof of residence, a copy of educational qualifications, proof of work and income, as well as a photo. Photocopy and/or any official documents provided by the applicant as evidence including job applications as well as images and sounds in job interviews for use in the selection process for the next step
3. กรณีบริษัทต้องการใช้ข้อมูลส่วนบุคคลของท่านนอกเหนือจากวัตถุประสงค์ที่ระบุไว้นี้ บริษัทจะแจ้งให้ทราบขอความยินยอมจากท่านเป็นกรณีไป / In the event that the company wants to use your personal information other than the purposes stated herein. The company will inform you of your consent on a case by case basis
4.บริษัทจะไม่เก็บรักษาข้อมูลส่วนบุคคล เกินกว่าระยะเวลาที่กฎหมายกำหนด และระยะเวลาการเรียกร้องสิทธิหรืออายุความฟ้องร้องดำเนินคดีในทางแพ่ง (10 ปี)  กรณีเป็นภาพเคลื่อนไหวจะเก็บไว้เป็นระยะเวลา 90 วัน / The company will not retain personal data for more than the period required by law and the period of claims or civil litigation period (10 years) in the case of moving images will be kept for a period of 90 days
5.การถอนความยินยอมจะถอนเมื่อใดก็ได้   เว้นแต่จะมีข้อจำกัดสิทธิการถอนความยินยอมตามที่กฎหมายกำหนดไว้ / Withdrawal of consent can be withdrawn at any time unless there are restrictions on the right to withdraw consent as required by law. *
